THC binds CB1 receptors. These receptors sit on neurons across the brain. Counts run high in the cortex, hippocampus, cerebellum, and basal ganglia. Binding changes how those cells signal. The result is the high: altered mood, memory, movement, and time sense. CBD does not bind CB1 with the same strength and does not produce that high.

Acute effects while high

Memory

The hippocampus stores new memories. THC disrupts that step. Users forget words, names, and details heard minutes earlier. Lab scores on working memory and verbal recall tests drop during intoxication and return after the drug clears.

Attention and reaction time

Divided attention slows. Reaction time lengthens. Driving tests after smoking show later braking and more lane drift. CDC lists cannabis among substances that impair driving.

Time and movement

Time estimates run short or long. Minutes can feel like hours. Fine motor control and balance drop, which raises the chance of a fall.

How long the effects last

  • Smoked or vaped: onset 1 to 10 minutes, peak at 15 to 30 minutes, fade over 1 to 3 hours.
  • Edibles: onset 30 to 60 minutes, peak at 2 to 3 hours, fade over 4 to 8 hours.

Brain effects track blood THC levels. THC also stores in fat tissue. Detectable THC on a test does not equal impairment.

Heavy or daily use

Daily users show lower CB1 receptor counts on PET scans. The brain cuts receptor numbers to adjust to constant THC. This downregulation affects memory and attention. Receptor counts return to baseline after about 2 to 4 weeks without cannabis. Most studies find that memory and attention gaps close after a month of abstinence. Heavy use is the main factor behind lasting deficits.

Tolerance and withdrawal

Tolerance develops with regular use. A user needs more THC for the same effect. Stopping after heavy use can cause irritability, sleep problems, low appetite, and cravings. Symptoms peak in the first week and ease over two weeks.

Adolescents and young adults

The prefrontal cortex keeps developing to about age 25. THC alters signaling in that region during this window. Studies link teen cannabis use to lower scores on memory and attention tests later in life. Some people recover. Others show a gap that holds.

Psychosis risk

High-THC products carry a higher risk of psychotic symptoms: paranoia, hallucinations, and confusion. People with a family history of schizophrenia or bipolar disorder face more risk. The link is stronger with daily use and high THC concentration. Researchers do not agree on whether cannabis causes schizophrenia or triggers it in people who already carry risk.

CBD

CBD does not cause the high. Animal studies show it blocks some THC effects. Human results are mixed. CBD may ease anxiety and, in some reports, blunt THC memory effects. A store label does not guarantee the CBD amount.

What the evidence does not settle

Research has not shown that cannabis kills brain cells in adults. Evidence on lasting IQ change is mixed, with heavy use as the key factor. Cognitive symptoms during the high are expected. Symptoms that last weeks after stopping warrant a doctor.
